Back on Track - NAPHL

At NAPHL we believe that mental health is just as important as physical health. Our philosophy is to provide personalised services that match your needs and help you to be the best person you can be.

 

Our team of highly trained clinicians offer a warm, welcoming, and non-judgemental environment where clients are supported to improve their mental health and wellbeing.  

 

Our mental health services cover all age groups including children, teenagers, adults and seniors. We can help with a wide range of issues from mild mental health concerns to more severe and complex issues such as anxiety and depression

Back on Track is an Alcohol and Other Drugs care coordination program that offers a single simple access point for young people to get support, information and referral to the relevant support services on offer in your region. 
 
With the vision to simplify and streamline access to relevant AOD support services, The Back on Track program care co-ordinator will meet with the young person and asses the type and level of support required and work with the young person to create a goal orientated plan to help them get back on track. 

 

The target clients for the program are:

  • Under 25 

  • A history of Alcohol and/or other Drug dependence or high use 

  • Aboriginal or Torres Strait Islander (does not exclude non-indigenous) 

  • Involvement with either juvenile or adult corrections 

  • Involvement with police 

  • At risk of re-offending or relapse into dependent behaviour 

  • A history of Out of Home Care (foster or kinship)
